1. Initial Assessment

I will meet with you and your loved one in the comfort of your home to gain a comprehensive understanding of your unique situation. This is an opportunity to share your story—your history, your goals, and what matters most to you. No detail is too small, as each small piece of information will help us to create a thoughtful and individualized plan.

During this visit, I will assess current abilities, identify evolving needs, and explore the types of support you and your family may wish to consider. Following the assessment, you will receive a detailed written summary outlining key findings and recommendations.

*This assessment serves as the foundation for all future services and is required to move forward with Personalized Care Matching.*

2. Care Readiness Plan
"The Roadmap"

This service is available following your initial complimentary phone consultation. After gathering a brief overview of your situation, CCSCA will develop a personalized, step-by-step action plan designed to help you move forward with clarity and confidence.

You will receive a prioritized checklist of tasks tailored to your specific needs, outlining what to address and in what order. When applicable, we will also include one vetted service provider recommendation for each step to help you begin taking action right away.

This roadmap is uniquely designed for your situation and serves as a practical starting point to help you prepare in advance—before a higher level of care is needed—so you can plan thoughtfully rather than react under pressure.

3. Personalized Care Matching

Following your initial assessment, if you choose to explore care options within the recommended level of care, CCSCA will develop a customized care matching report tailored to your needs. We thoughtfully vet and evaluate local care providers, presenting you with a curated selection of options along with the key information you need to make informed decisions. This process allows you to move forward with confidence in selecting and interviewing providers best suited to support your loved one.

4. Hourly Consulting 

For families who may not need a full care plan or provider matching service, hourly consultation offers flexible, as-needed support. This option is ideal if you simply need occasional guidance, a point in the right direction, or assistance navigating decisions as they arise.

Support is available at your own pace, allowing you to reach out when questions come up or circumstances change. Whether you need help interpreting options, understanding next steps, or talking through a specific concern, this service provides expert insight without the commitment of a comprehensive plan.
